We need a full-time Social Services Designee, to join our team at Indianspring Skilled Nursing and Rehabilitation in Cincinnati, OH .You’ll assisted our Licensed social Worker by ensuring that the medically-related emotional and social needs of our residents are individually met and maintained. $15-$18.50 Per Hour You’ll Be Responsible For: Assisting with gathering information from residents and family membersAssisting with the completion of the Integrated Personal Services Assessment and MDSAssisting with the discharge planning process by arranging community resources, alternative placement, etc.Attending Care Conferences as directed by the LSWAssisting LSW with any other case management tasks deemed necessary to the Department.Fulfilling basic documentation requirements for social services.Assisting (after training) in the BIMS and the PHQ-9 You’ll Need To Have: An STNA certification or eligibility and experience in long term care.Excellent communication skillsA High School Diploma and/or GEDHealthcare industry, social work, or other related experienceThe ability to multi-task in a fast-paced, high volume environmentExceptional organization skills with strong attention to detailA personable and professional demeanorComputer proficiency in Excel, Word, Power Point, and Outlook We’re Proud To Offer You: Paid Time Off Paid Training Health Insurance Dental Insurance Vision Insurance Life Insurance Disability Insurance 401K with Employer Match Flexible Schedule Education Scholarships Continuing Education Credits Employee Discounts Exceptional Care & Quality Service are the hallmarks of Indianspring's in-house team of talented and experienced licensed therapists, nurses and caregivers .
