The job you have been looking for! AuthoraCare Collective is currently seeking a Social Worker Clinical Manager . This position is Full--Time , Monday through Friday. Job Summary: Has shared authority and responsibility for administration of social work interventions for Home Care, Long Term Care, and IPU patients in conjunction with the Director of Social Work. Plans, organizes, and leads best practice standards of professional social work. Ensures professional standards and compliance requirements are met with local, state, and federal licensing and credentialing agencies. Shares responsibility for serving as a clinical resource to the clinical staff. Conducts documentation and quality monitoring audits. Is accountable for social work caseloads, assigning, and rebalancing, when necessary. A significant responsibility includes the orientation process for new staff, while maintaining a minimal case load of 5-8 patients when needed during vacancies, staff turnover, and/or organizational growth. Will attend and/or support weekly IDT meetings. Will facilitate, attend, and/or support weekly IDT meetings for assigned teams in coordination with nursing leadership partners. Education/Experience: Masters graduate of an accredited School of Social Work Must be currently licensed LCSW in North Carolina and in good standing with licensing board. Three to five years' experience in social work as LCSW providing therapeutic interventions, preferably in hospice, private practice, or healthcare setting required. Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation. Vaccination against COVID-19 is mandatory at our organization unless you are approved for an accommodation due to a religious objection or ADA covered medical condition. The organization will also review medical recommendations for a delay in vaccination or for other contraindications to vaccination.
